Causa una enfermedad exantem\u00e1tica infantil de curso benigno y autolimitado en la mayor\u00eda de los casos, denominada varicela. Tras la primoinfecci\u00f3n, el virus puede permanecer latente en los ganglios dorsales y craneales y reactivarse generando una erupci\u00f3n vesicular acompa\u00f1ada de dolor neur\u00edtico de distribuci\u00f3n dermatom\u00e9rica llamado z\u00f3ster. Esta reactivaci\u00f3n tambi\u00e9n puede implicar manifestaciones neurol\u00f3gicas (meningitis, encefalitis, par\u00e1lisis de los nervios craneales, vasculopat\u00eda), cuya incidencia aumenta con la edad y el descenso de la inmunidad celular, siendo poco com\u00fan en inmunocompetentes. Objetivos: Difundir informaci\u00f3n acerca de una enfermedad potencialmente tratable cuya sospecha diagn\u00f3stica suele ser baja.
Material y m\u00e9todos: se realiz\u00f3 un estudio retrospectivo, observacional y descriptivo en que se incluyeron 11 historias cl\u00ednicas de pacientes internados en el Sanatorio Parque desde enero de 2020 a agosto de 2024 diagnosticados con meningoencefalitis por virus varicela z\u00f3ster mediante m\u00e9todos moleculares.
Resultados: El 27 % de los pacientes presentaban una causa de inmunosupresi\u00f3n subyacente. De estos, el 66 % de los pacientes con trasplante de \u00f3rgano s\u00f3lido. La resonancia magn\u00e9tica no aport\u00f3 ninguna herramienta a la hora de arribar al diagn\u00f3stico. El 91 % de los pacientes fueron diagnosticados mediante el m\u00e9todo de Filmarray<\/em>.

Introducci\u00f3n<\/h3>\n
El VVZ es el de menor incidencia. Esto sumado a su presentaci\u00f3n inespec\u00edfica y falta de disponibilidad de pruebas llevan a que el diagn\u00f3stico requiera un alto \u00edndice de sospecha. Los s\u00edntomas neurol\u00f3gicos aislados que se presentan sin erupci\u00f3n pueden pasarse por alto f\u00e1cilmente una vez que se descarta la meningitis por virus herpes simple y pueden dar lugar a la interrupci\u00f3n prematura de los antivirales.\u00a0<\/p>\nObjetivos<\/h3>\n
Optimizar la utilizaci\u00f3n de m\u00e9todos espec\u00edficos para un diagn\u00f3stico precoz y tratamiento oportuno.
Detallar efectos adversos secundarios al tratamiento y complicaciones en relaci\u00f3n a la infecci\u00f3n.

Resultados<\/h3>\n
El 91 % de los pacientes fueron diagnosticados mediante t\u00e9cnica de Filmarray<\/em>. La mortalidad fue del 18\u00a0%. La resonancia magn\u00e9tica no result\u00f3 un m\u00e9todo diagn\u00f3stico sensible, ya que no se vieron alteraciones en ninguno de los pacientes.
El 27 % de los pacientes analizados presentaron una causa de inmunosupresi\u00f3n subyacente (18\u00a0% trasplante renal, 9 % c\u00e1ncer activo). En promedio, el tiempo de internaci\u00f3n fue de 7,6 d\u00edas.
La principal complicaci\u00f3n fue la aparici\u00f3n de falla renal aguda (54 %); en promedio, luego de 3 d\u00edas de tratamiento con aciclovir endovenoso. La complicaci\u00f3n m\u00e1s grave fue el deterioro del sensorio con necesidad de intubaci\u00f3n orotraqueal, en el 18 % de los pacientes.<\/p>\nDiscusi\u00f3n<\/h3>\n
El exantema del z\u00f3ster suele acompa\u00f1ar a la enfermedad neurol\u00f3gica, excepto en pacientes inmunodeprimidos en los que suele estar ausente. Sin embargo, se han descrito casos de meningitis por VVZ en inmunocompetentes, sin exantema, ya que se cree que el virus reactivado podr\u00eda circular directamente al sistema nervioso central (SNC), sin viajar a la epidermis.
Desde el punto de vista cl\u00ednico la meningoencefalitis por VVZ puede presentarse con cefalea intensa, v\u00f3mitos, fiebre, alteraci\u00f3n del sensorio, focalidad neurol\u00f3gica, convulsiones, entre otros. Al ser las manifestaciones cl\u00ednicas inespec\u00edficas y estar presentes en otras causas de encefalitis obliga al personal de salud a considerar la meningoencefalitis por varicela z\u00f3ster como diagn\u00f3stico diferencial.
Las caracter\u00edsticas del l\u00edquido cefalorraqu\u00eddeo en la meningitis por VVZ son indistinguibles de otras meningitis virales, pero ha sido descrita la hipoglucorraquia en casos de meningitis por este virus en ni\u00f1os y adultos inmunocompetentes. La prueba de Reacci\u00f3n en Cadena de la Polimerasa (PCR) para VVZ en l\u00edquido cefalorraqu\u00eddeo, con o sin exantema, es esencial en el diagn\u00f3stico. Sin embargo, la introducci\u00f3n de nuevos m\u00e9todos moleculares como el Panel Meningitis\/Encefalitis FilmArray<\/em> facilit\u00f3 el arribo a un diagn\u00f3stico m\u00e1s certero. Este \u00faltimo es un sistema automatizado con t\u00e9cnica de PCR-m\u00faltiple, que mediante una \u00fanica muestra permite la identificaci\u00f3n de los 14 pat\u00f3genos m\u00e1s frecuentes implicados en esta patolog\u00eda. La muestra debe ser obtenida a trav\u00e9s de una punci\u00f3n lumbar que, a pesar de considerarse una t\u00e9cnica m\u00ednimamente invasiva, requiere de cierta destreza y entrenamiento por parte del operador. Adem\u00e1s, solo los centros de alta complejidad tienen acceso r\u00e1pido a estos nuevos m\u00e9todos moleculares, y su alto costo puede significar un problema para el sistema de salud. En general, no se observan alteraciones imagenol\u00f3gicas en las infecciones del sistema nervioso central por VVZ.
El tratamiento de elecci\u00f3n es el aciclovir durante 10-14 d\u00edas para encefalitis VVZ en pacientes inmunosuprimidos y 7 d\u00edas para pacientes inmunocompetentes. Se ha descripto una mortalidad del\u00a09-20\u00a0%, pudiendo evolucionar con secuelas leves a severas en un 15\u00a0%.1-4<\/sup><\/p>\nConclusiones<\/h3>\n
