Por presentar empeoramiento de su disnea y hemoptisis se decidi\u00f3 su internaci\u00f3n en UCI. El manejo de la hemoptisis masiva a trav\u00e9s de fibrobroncoscopia tiene 3 objetivos, que son la estabilidad hemodin\u00e1mica, aislamiento pulmonar y control del sangrado.6 La hemoptisis puede ser un signo cl\u00ednico com\u00fan de resoluci\u00f3n ambulatoria y programada, sin embargo, los casos graves, donde la permeabilidad de la v\u00eda a\u00e9rea este comprometida y consigo la ventilaci\u00f3n, la vida del paciente puede encontrarse en verdadero peligro. En este caso, el equipo de UCI, al contar con personal especializado en cuidados respiratorios y cirug\u00eda tor\u00e1cica disponible las 24 horas del d\u00eda, comprendi\u00f3 y ejecut\u00f3 la resoluci\u00f3n del cuadro. Introducci\u00f3n<\/h3>\n
Dentro de las etiolog\u00edas m\u00e1s frecuente se encuentran el c\u00e1ncer de pulm\u00f3n 30,3 %, bronquiectasias 27,9 %, infecci\u00f3n del tracto respiratorio inferior 4,2 %, embolia pulmonar 3,6 %, causa iatrog\u00e9nica 3,6 %, malformaci\u00f3n arterio venosa 3,6 %.2
Es de suma importancia el diagn\u00f3stico e interpretaci\u00f3n cl\u00ednico precoz, ya que obtener r\u00e1pidamente la etiolog\u00eda y localizaci\u00f3n del sangrado ser\u00e1 clave en los casos graves, donde la necesidad de unidad de cuidados intensivos (UCI) e intubaci\u00f3n orotraqueal (IOT) ser\u00e1n m\u00e1s probables, debido a que la mortalidad intrahospitalaria puede alcanzar hasta un 27 %.1,2 Dada la severidad del cuadro es necesario contar con procedimientos quir\u00fargicos de urgencia que puedan resolver la obstrucci\u00f3n. La broncoscopia consiste en la introducci\u00f3n de un tubo con iluminaci\u00f3n distal y capacidad de extraer cuerpos extra\u00f1os de la v\u00eda \u00e1rea, puede realizarse con un broncoscopio flexible o r\u00edgido.3,4 Dicho procedimiento ha demostrado ser el mas confiable en localizar el origen del sangrado en comparaci\u00f3n a la radiograf\u00eda y tomograf\u00eda de t\u00f3rax, se presenta como opci\u00f3n segura para realizar al pie de la cama del paciente en la UCI, por lo que ser\u00e1 de importancia en los casos de hemoptisis masiva que necesiten evaluar y resolver una obstrucci\u00f3n significativa de la v\u00eda a\u00e9rea.2,5<\/p>\nObjetivos<\/h3>\n
Presentaci\u00f3n del caso<\/h3>\n
\nEn su ingreso a la unidad cr\u00edtica se observ\u00f3 taquipnea, taquicardia, uso de m\u00fasculos accesorios ventilatorios, se realiz\u00f3 un estado \u00e1cido base arterial denotando hipoxemia y acidosis respiratoria, por lo cual se decidi\u00f3 la IOT, posterior a dicho procedimiento el paciente fue soportado a trav\u00e9s de asistencia mec\u00e1nica invasiva (AVM), generando hipoventilaci\u00f3n por impedancia traqueobronquial evidente, se realizaron m\u00faltiples procedimientos de aspiraci\u00f3n traqueal a trav\u00e9s del tubo orotraqueal, aspirando abundante cantidad de contenido hem\u00e1tico, pero no se logr\u00f3 una adecuada permeabilidad de la v\u00eda a\u00e9rea, lo cu\u00e1l gener\u00f3 empeoramiento de hipoxemia y acidosis respiratoria e inestabilidad hemodin\u00e1mica, con requerimiento de reposici\u00f3n de gl\u00f3bulos rojos, expansi\u00f3n a trav\u00e9s de ringer lactato e inotr\u00f3picos.
\nSe decidi\u00f3 la realizaci\u00f3n de una fibrobroncoscop\u00eda al pie de la cama, visualizando moldes endobronqueales hem\u00e1ticos, los cuales obstru\u00edan de manera casi total el calibre de la v\u00eda a\u00e9rea. Posterior a la realizaci\u00f3n del procedimiento, mejor\u00f3 la ventilaci\u00f3n y la oxigenaci\u00f3n del paciente, este episodio vuelve a repetirse, por lo cu\u00e1l se decidi\u00f3 procedimiento a trav\u00e9s de fibrobronscop\u00eda r\u00edgida en quir\u00f3fano. Se observa en Figura 1 la obstrucci\u00f3n generada en la v\u00eda a\u00e9rea, y en la Figura 2 la extracci\u00f3n del molde.
\nEl molde endobronquial retirado se muestra en la Figura 3. Fue enviado al servicio anatomopatol\u00f3gico, los cuales informaron, molde fibrohematico que incluye c\u00e9lulas epiteliales aisladas sin reconocimiento de c\u00e9lulas neopl\u00e1sicas.
\nUna vez admitido en UCI posterior al procedimiento quir\u00fargico el paciente mejor\u00f3 notablemente el comportamiento ventilatorio, hemodin\u00e1mico y de estado \u00e1cido base, al d\u00eda siguiente se realiz\u00f3 vacaciones de sedaci\u00f3n, y se desvincul\u00f3 exitosamente de la AVM. \nLos grandes vol\u00famenes de sangre perdidos deben repletarse r\u00e1pidamente con el fin de evitar isquemia tisular, shock irreversible y falla multiorg\u00e1nica. La primer l\u00ednea de tratamiento son la administraci\u00f3n de cristaloides, ya que, los coloides no han demostrado mejores desenlaces en mortalidad, falla multiorg\u00e1nica, d\u00edas en UCI o en hospital, d\u00edas de AVM o de terapia de reemplazo renal.7,8,9 En nuestro caso se utiliz\u00f3 ringer lactato.
\nPara ubicar el sitio de sangrado, los signos cl\u00ednicos pueden ser inespec\u00edficos y la aspiraci\u00f3n a trav\u00e9s de cat\u00e9ter de aspiraci\u00f3n K 32 puede ser insuficiente, el broncoscopio r\u00edgido tiene mayor di\u00e1metro, lo cual tiene como ventaja mayor poder de succi\u00f3n y mantenimiento de la v\u00eda a\u00e9rea permeable, es de elecci\u00f3n en caso de sangrado proximal de la v\u00eda a\u00e9rea, pero no en sangrado perif\u00e9rico, raz\u00f3n por la cual el fibrobroncoscopio flexible es el ideal en estos casos; sin embargo, su menor calibre limita las posibles intervenciones terap\u00e9uticas y dificulta una adecuada succi\u00f3n en hemoptisis masiva.10
\nEn este caso, no se utiliz\u00f3 terapia antifibrinol\u00edtica (\u00e1cido tranex\u00e1mico) para el control de hemoptisis. La literatura revisada es escasa, no es comparable y tiene un bajo nivel de evidencia.11 Hay evidencia insuficiente para saber si la terapia antifibrinol\u00edtica debe ser usada en el tratamiento de hemoptisis, aunque algunos estudios indican una reducci\u00f3n de la duraci\u00f3n del sangrado.12
\nEl manejo de la hemoptisis masiva debe ser multidisciplinario (terapista, kinesi\u00f3logo respiratorio, cirujano de t\u00f3rax y anestesi\u00f3logo). \nEs de suma importancia encontrar el nivel de obstrucci\u00f3n, puesto que se pueden tomar medidas err\u00f3neas para intentar resolver el caso, como sospechar de v\u00eda a\u00e9rea artificial mal posicionada, fugas en el circuito, obstrucci\u00f3n de v\u00edas a\u00e9reas de conducci\u00f3n o incluso un error t\u00e9cnico en el respirador. En este caso, al descartar las situaciones mencionadas anteriormente, se comprendi\u00f3 la necesidad de realizar de urgencia una videobroncoscopia para identificar el sitio y retirar la masa obstructiva.
